Discuss and analyze the pre-clinical study requirements mandated by FDA.

10 marks

Answer

Pre-clinical studies are essential for establishing the safety profile of a drug candidate before human exposure. The FDA mandates these studies to evaluate pharmacological activity, toxicity, and pharmacokinetic properties. Key requirements include:

  1. Pharmacology: Studies must demonstrate the mechanism of action and the primary pharmacodynamic effects of the drug in relevant animal models.
  2. Toxicology: Comprehensive assessment of acute, sub-chronic, and chronic toxicity is required. This includes determining the No-Observed-Adverse-Effect-Level (NOAEL) and identifying target organ toxicity.
  3. Safety Pharmacology: Specific assessments of the cardiovascular, respiratory, and central nervous systems are mandatory to identify potential life-threatening adverse effects.
  4. Pharmacokinetics and Toxicokinetics: Evaluation of absorption, distribution, metabolism, and excretion (ADME) is necessary to understand drug exposure levels in relation to observed toxic effects.
  5. Genotoxicity and Carcinogenicity: Standard batteries of tests (e.g., Ames test, chromosomal aberration assays) are required to assess mutagenic potential. Long-term carcinogenicity studies are typically initiated before Phase III trials.
  6. Reproductive Toxicology: Assessment of effects on fertility, embryo-fetal development, and pre- and post-natal development is required for drugs intended for long-term use or specific patient populations.

All pre-clinical studies must be conducted in compliance with Good Laboratory Practice (GLP) regulations to ensure the quality, integrity, and reliability of the data submitted in the Investigational New Drug (IND) application.

b

Write the regulatory overview of Phase-I trials. Explain traditional 3+3 design of dose escalation studies.

5 marks

Answer

Phase-I clinical trials represent the first administration of an investigational drug to humans, primarily focusing on safety, tolerability, and pharmacokinetics rather than efficacy. These trials are conducted in a small group of healthy volunteers (typically 20-80) to determine the Maximum Tolerated Dose (MTD) and identify dose-limiting toxicities (DLT).

The 3+3 design is the traditional rule-based approach for dose escalation:

  1. Start by enrolling a cohort of 3 patients at the lowest dose level.
  2. If 0 out of 3 patients experience a DLT, the dose is escalated to the next level.
  3. If 1 out of 3 patients experiences a DLT, 3 additional patients are enrolled at the same dose level.
  4. If 0 out of 3 (in the expanded cohort) experience a DLT, the dose is escalated. If 2 or more experience a DLT, the dose is considered to have exceeded the MTD, and the previous dose level is declared the MTD.
  5. If 2 or more patients experience a DLT in the initial group of 3, the dose is deemed too toxic, and the trial stops or de-escalates.
