What are Communicable diseases? Explain briefly the role of a Community Pharmacist in preventing them by proper family planning and balanced diet.

14 marks

Answer

Communicable diseases are infectious illnesses caused by pathogenic microorganisms, such as bacteria, viruses, parasites, or fungi, that can spread directly or indirectly from one person to another, from animals to humans, or through contaminated environments. Transmission occurs via contact with infected individuals, bodily fluids, contaminated surfaces, insect vectors, or ingestion of contaminated food and water.

The Community Pharmacist plays a vital role in disease prevention through the following strategies:

1. Role in Family Planning:

  • Providing education on the use of barrier contraceptives, such as condoms, which are essential in preventing the transmission of sexually transmitted infections (STIs) and HIV.
  • Counseling patients on reproductive health and the importance of spacing pregnancies, which improves maternal and child health outcomes, thereby increasing overall immunity and resilience against infectious diseases.
  • Ensuring access to safe and effective contraceptive methods to reduce high-risk behaviors that may lead to the spread of communicable diseases.

2. Role in Promoting a Balanced Diet:

  • Educating the community on the relationship between nutrition and immune function; a well-nourished body is significantly more capable of resisting and recovering from infections.
  • Advising on the intake of essential vitamins (e.g., Vitamin C, D, and Zinc) that support immune system integrity, helping to reduce the severity and duration of common communicable illnesses.
  • Promoting food safety and hygiene practices to prevent foodborne illnesses, which are a major category of communicable diseases.
  • Identifying signs of malnutrition in patients and referring them to appropriate healthcare providers, as malnourished individuals are highly susceptible to opportunistic infections like tuberculosis.

By acting as a primary point of contact, the pharmacist serves as a key educator, ensuring that patients understand how lifestyle choices, such as family planning and proper nutrition, directly contribute to the prevention and control of communicable disease outbreaks within the community.

2

What is Patho-physiology and describe the treatment options for Diarrhoea and Constipation.

14 marks

Answer

Pathophysiology is the study of the disordered physiological processes associated with disease or injury. It bridges the gap between basic science and clinical medicine by explaining the functional changes that occur in the body due to a pathological condition.

Diarrhoea is characterized by an increase in stool frequency, fluidity, or volume. Its pathophysiology involves four main mechanisms: 1) Osmotic diarrhoea, caused by the presence of non-absorbable solutes in the gut lumen; 2) Secretory diarrhoea, resulting from increased intestinal secretion of electrolytes and water; 3) Exudative diarrhoea, due to inflammation or ulceration of the intestinal mucosa; and 4) Motility disorders, where rapid transit prevents adequate fluid absorption. Treatment options include:

  • Oral Rehydration Therapy (ORT) to replace fluids and electrolytes.
  • Antimotility agents like loperamide to slow intestinal transit.
  • Adsorbents such as kaolin-pectin to bind toxins.
  • Probiotics to restore intestinal flora.
  • Antibiotics if a specific bacterial infection is confirmed.

Constipation is defined as infrequent or difficult bowel movements. Its pathophysiology often involves slow colonic transit time, pelvic floor dysfunction, or inadequate dietary fiber and fluid intake, leading to excessive water reabsorption and hardened stools. Treatment options include:

  • Lifestyle modifications: Increasing dietary fiber intake and daily fluid consumption.
  • Bulk-forming laxatives (e.g., psyllium) to increase stool volume and stimulate peristalsis.
  • Osmotic laxatives (e.g., lactulose, magnesium hydroxide) to draw water into the bowel.
  • Stimulant laxatives (e.g., bisacodyl, senna) to increase intestinal motility.
  • Stool softeners (e.g., docusate sodium) to ease passage.
  • Enemas or suppositories for acute relief.
