a

Discuss the role of beta blockers in Heart Failure.

7 marks

Answer

Beta-blockers are foundational therapy in chronic heart failure with reduced ejection fraction (HFrEF). They function by antagonizing the deleterious effects of chronic sympathetic nervous system activation. Key roles include:

  • Reduction of Myocardial Oxygen Demand: By decreasing heart rate and contractility, beta-blockers reduce cardiac workload.
  • Anti-arrhythmic Effects: They protect the myocardium from the pro-arrhythmic and toxic effects of catecholamines, reducing the risk of sudden cardiac death.
  • Reverse Remodeling: Long-term therapy improves left ventricular systolic function and geometry by inhibiting the signaling pathways that lead to fibrosis and apoptosis.
  • Clinical Benefit: Evidence-based agents like carvedilol, metoprolol succinate, and bisoprolol significantly reduce hospitalizations and mortality.

Treatment must be initiated at low doses in clinically stable, euvolemic patients and titrated slowly to target doses, as acute initiation can temporarily worsen heart failure symptoms.

b

Describe briefly the management of ST segment elevation Myocardial Infarction.

7 marks

Answer

The management of ST-segment elevation myocardial infarction (STEMI) focuses on immediate reperfusion and secondary prevention. The primary goal is to restore coronary blood flow as rapidly as possible.

  • Immediate Pharmacotherapy: Patients receive aspirin (chewed), a P2Y12 inhibitor (e.g., ticagrelor or clopidogrel), and anticoagulation (e.g., heparin or enoxaparin) to prevent further thrombus propagation. Nitroglycerin, morphine, and oxygen are used for symptom relief and hemodynamic stabilization.
  • Reperfusion Therapy: Primary Percutaneous Coronary Intervention (PCI) is the preferred strategy if it can be performed within 90 minutes of medical contact. If PCI is unavailable within the recommended timeframe, fibrinolytic therapy (e.g., alteplase, tenecteplase) should be administered within 30 minutes of arrival.
  • Post-MI Management: Long-term therapy includes the initiation of beta-blockers, ACE inhibitors or ARBs, and high-intensity statins to prevent remodeling and future events.
  • Risk Assessment: Continuous monitoring for complications such as arrhythmias, heart failure, or mechanical defects is essential during the acute phase.
