Answer any five questions. All questions carry equal marks.
Main
Mention the need for conducting the bioavailability studies. Explain the methods for determination of bioavailability.
Answer
Bioavailability studies are essential to ensure that a drug product delivers the active pharmaceutical ingredient to the systemic circulation at the intended rate and extent. The primary needs for conducting these studies include: establishing the bioequivalence of generic formulations to reference products, assessing the impact of formulation changes or manufacturing variations on drug performance, determining the influence of physiological factors like food or gastric pH on absorption, and guiding dosage regimen adjustments for drugs with narrow therapeutic indices.
The methods for determining bioavailability are broadly classified into pharmacokinetic and pharmacodynamic approaches:
- Plasma Level-Time Studies: This is the most common and accurate method. It involves measuring the concentration of the drug in plasma at various time intervals after administration. Key parameters calculated include the area under the curve (AUC), which represents the total extent of absorption, peak plasma concentration (Cmax), and time to reach peak concentration (Tmax).
- Urinary Excretion Studies: This method is used when the drug is excreted primarily in unchanged form in the urine. It is based on the principle that the amount of drug excreted is proportional to the amount absorbed. Parameters measured include the cumulative amount of drug excreted (Du) and the rate of urinary excretion (dDu/dt). This method is non-invasive but requires complete urine collection.
- Pharmacodynamic Methods: These are used when plasma or urine concentration data cannot be obtained or are not reflective of the therapeutic effect. This involves measuring a direct pharmacological response, such as changes in blood pressure, heart rate, or blood glucose levels, over time. While clinically relevant, these methods are often less precise due to high inter-subject variability in physiological response.
- Clinical Observations: In cases where other methods are not feasible, bioavailability may be inferred from clinical evidence of therapeutic efficacy or toxicity. This is the least quantitative method and is generally reserved for drugs where precise pharmacokinetic monitoring is not possible.
Define compartment. Mention the advantages of compartment modelling.
Answer
A compartment is defined as a kinetically homogeneous unit or a group of tissues that have similar blood flow and affinity for a drug, such that the drug concentration within the unit is assumed to be uniform at any given time. It is a mathematical abstraction rather than a physiological or anatomical entity.
The advantages of compartment modelling include:
- It simplifies complex biological systems into manageable mathematical structures.
- It allows for the prediction of drug concentrations in plasma and tissues over time.
- It facilitates the calculation of pharmacokinetic parameters such as elimination rate constant, volume of distribution, and clearance.
- It provides a basis for designing dosage regimens and adjusting doses for patients.
