1

Describe the scope and status of clinical pharmacy in India.

14 marks

Answer

Clinical pharmacy in India has evolved from a hospital-based dispensing role to a patient-centered practice focused on optimizing drug therapy and improving health outcomes. Its scope encompasses medication therapy management, patient counseling, adverse drug reaction monitoring, and therapeutic drug monitoring.

The scope of clinical pharmacy practice in India includes:

  • Medication Therapy Management: Reviewing patient medication profiles to identify, prevent, and resolve drug-related problems such as drug interactions, therapeutic duplications, and inappropriate dosing.
  • Patient Counseling: Educating patients on the correct use of medications, lifestyle modifications, and the importance of adherence to improve therapeutic compliance.
  • Pharmacovigilance: Actively participating in the detection, assessment, and reporting of adverse drug reactions to ensure patient safety within hospital settings.
  • Therapeutic Drug Monitoring: Collaborating with clinicians to adjust dosages based on serum drug concentrations, particularly for drugs with narrow therapeutic indices.
  • Drug Information Services: Providing evidence-based information to healthcare professionals regarding drug selection, dosage, and safety.

The status of clinical pharmacy in India is characterized by:

  • Academic Foundation: The introduction of the Pharm. D program has significantly advanced the clinical role of pharmacists by providing rigorous training in clinical skills, pathophysiology, and therapeutics.
  • Hospital Integration: While well-established in large corporate and tertiary care hospitals, clinical pharmacy services are still in the developmental phase in smaller private and government hospitals.
  • Regulatory Support: The Pharmacy Council of India (PCI) has recognized the role of clinical pharmacists, though formal integration into the national healthcare workforce and standardized job descriptions across all states remain ongoing challenges.
  • Collaborative Practice: There is an increasing acceptance of clinical pharmacists as members of the multidisciplinary healthcare team, particularly in intensive care units and specialized departments like oncology and nephrology.

In conclusion, while clinical pharmacy is gaining momentum in India, its full potential is yet to be realized through broader policy implementation and the formal recognition of the clinical pharmacist's role in the public health sector.

2

Discuss the scales to measure causality assessment? Explain the two major predisposing factors of ADRs.

14 marks

Answer

Causality assessment is the process of determining the likelihood that a specific drug caused an observed adverse clinical event. Several standardized scales are utilized in clinical practice and pharmacovigilance to categorize this relationship.

Scales for Causality Assessment:

  1. Naranjo Algorithm: The most widely used scale, consisting of 10 structured questions. It assigns numerical scores to factors such as temporal sequence, dechallenge, rechallenge, and alternative causes. Scores are categorized as: Definite (≥9), Probable (5-8), Possible (1-4), or Doubtful (≤0).
  2. WHO-UMC Causality Categories: A qualitative system based on clinical judgment. It classifies ADRs into: Certain, Probable/Likely, Possible, Unlikely, Conditional/Unclassified, and Unassessable/Unclassifiable.
  3. Liverpool Causality Assessment Tool (LCAT): A structured, evidence-based approach that uses a decision tree to assess the relationship, often used in pediatric settings.
  4. The French Method (Bégaud et al.): A two-step process evaluating both intrinsic (chronological and semiological) and extrinsic (bibliographic) data to classify causality.

Predisposing Factors of ADRs:

Adverse Drug Reactions (ADRs) are influenced by various patient-specific and drug-specific variables. The two major categories of predisposing factors are:

  1. Patient-Related Factors (Intrinsic): These include age (extremes of age, such as neonates and the elderly, are at higher risk due to altered pharmacokinetics), gender, genetic polymorphisms (e.g., variations in CYP450 enzymes affecting drug metabolism), presence of underlying diseases (especially hepatic or renal impairment), and history of previous allergies or hypersensitivity.
  2. Drug-Related Factors (Extrinsic): These include the pharmacological profile of the drug (e.g., narrow therapeutic index), the dosage and duration of therapy, the route of administration, and the potential for drug-drug or drug-food interactions. Polypharmacy is a significant extrinsic risk factor, as it increases the probability of adverse interactions and medication errors.
3
