Explain the pathogenesis, symptoms and management of Alzheimer’s disease.
Answer
Pathogenesis: Alzheimer’s disease is primarily driven by the accumulation of extracellular amyloid-beta (Aβ) plaques and intracellular neurofibrillary tangles composed of hyperphosphorylated tau protein. This leads to synaptic dysfunction, loss of cholinergic neurons, and widespread neurodegeneration, particularly in the hippocampus and cerebral cortex. Chronic neuroinflammation and oxidative stress further exacerbate neuronal death.
Symptoms: The clinical presentation follows a progressive decline in cognitive and functional abilities:
- Early stage: Short-term memory loss, difficulty learning new information, and mild word-finding problems.
- Middle stage: Disorientation to time and place, impaired executive function, personality changes, and behavioral disturbances (agitation, wandering).
- Late stage: Severe cognitive impairment, loss of verbal communication, physical decline (incontinence, dysphagia), and total dependence for activities of daily living.
Management: Treatment focuses on symptom management and improving quality of life, as there is currently no cure.
- Pharmacotherapy: Acetylcholinesterase inhibitors (e.g., Donepezil, Rivastigmine, Galantamine) are used for mild to moderate cases to increase synaptic acetylcholine levels. NMDA receptor antagonists (e.g., Memantine) are used in moderate to severe stages to reduce glutamate-induced excitotoxicity.
- Non-pharmacological management: Cognitive stimulation therapy, structured routines, and environmental modifications to ensure safety and reduce confusion.
- Supportive care: Management of comorbid conditions, nutritional support, and caregiver education to address the psychological and physical burden of caregiving.
Write the pathogenesis and management of migraine headache.
Answer
The pathogenesis of migraine is complex, primarily involving the trigeminovascular system. It is initiated by the activation of trigeminal nerve afferents, which release vasoactive neuropeptides such as calcitonin gene-related peptide (CGRP), substance P, and neurokinin A. This results in neurogenic inflammation, plasma protein extravasation, and vasodilation of intracranial blood vessels. Cortical spreading depression (CSD), a wave of neuronal depolarization followed by suppression, is believed to trigger the aura phase.
Management involves both non-pharmacological and pharmacological strategies:
- Non-pharmacological: Identification and avoidance of triggers (e.g., stress, sleep deprivation, specific foods), regular sleep patterns, and relaxation techniques.
- Abortive Therapy: Used for acute attacks. First-line agents include simple analgesics (NSAIDs like ibuprofen or naproxen) or acetaminophen for mild-to-moderate attacks. For moderate-to-severe attacks, triptans (e.g., sumatriptan) are the gold standard, acting as 5-HT1B/1D receptor agonists. Ergot alkaloids are alternatives for refractory cases.
- Prophylactic Therapy: Indicated for frequent or disabling attacks. Options include beta-blockers (propranolol), anticonvulsants (topiramate, valproate), and tricyclic antidepressants (amitriptyline). CGRP antagonists (monoclonal antibodies like erenumab) are newer options for chronic migraine prevention.
