Answer any five questions. All questions carry equal marks.
Main
What is clinical pharmacokinetics? Write its applications.
Answer
Clinical pharmacokinetics is the application of pharmacokinetic principles to the safe and effective therapeutic management of individual patients. It involves the study of the time course of drug absorption, distribution, metabolism, and excretion (ADME) to optimize drug therapy by adjusting dosage regimens based on patient-specific physiological and pathological factors.
Its primary applications include:
- Designing individualized dosage regimens based on patient parameters such as age, weight, renal function, and hepatic status.
- Monitoring drug concentrations in plasma (Therapeutic Drug Monitoring) to ensure levels remain within the therapeutic window.
- Adjusting therapy for patients with organ dysfunction (e.g., renal failure) to prevent toxicity or sub-therapeutic effects.
- Identifying and managing clinically significant drug-drug interactions.
- Evaluating the pharmacokinetics of drugs in special populations, such as pediatrics, geriatrics, and pregnant patients.
- Optimizing drug delivery systems to improve patient compliance and therapeutic outcomes.
How will you determine frequency of drug administration?
Answer
The frequency of drug administration is determined by the drug's half-life (t1/2) and the desired therapeutic range. The goal is to maintain steady-state plasma concentrations within the therapeutic window while minimizing fluctuations between peak and trough levels.
The determination process involves the following steps:
- Determine the Elimination Half-life: Calculate the t1/2 using the formula t1/2 = 0.693 / kwhere k is the elimination rate constant.
- Define the Therapeutic Window: Identify the Minimum Effective Concentration (MEC) and the Minimum Toxic Concentration (MTC).
- Calculate the Dosing Interval (τ): The interval is selected such that the peak concentration (Cmax) does not exceed the MTC and the trough concentration (Cmin) does not fall below the MEC. This is calculated using the accumulation factor and the desired peak/trough ratios.
- Adjust for Patient Factors: Modify the interval based on the patient's clearance (CL) and volume of distribution (Vd). For drugs with a narrow therapeutic index, more frequent dosing or controlled-release formulations may be required to reduce the fluctuation index.
- Steady-State Considerations: Ensure the interval allows the drug to reach steady state, typically achieved after 4 to 5 half-lives of the chosen dosing regimen.
