Discuss the process of drug discovery. Describe various approaches for drug discovery. Give the importance of pharmacological studies in drug discovery.

14 marks

Answer

Drug discovery is a systematic, multi-disciplinary process aimed at identifying new chemical entities (NCEs) or biological molecules that can effectively treat or prevent diseases. The process typically spans 10 to 15 years and involves several sequential stages.

The process begins with target identification and validation, where a biological target (e.g., a protein, receptor, or gene) associated with a disease is identified. Once validated, hit identification occurs through high-throughput screening of large chemical libraries. Promising hits are then optimized into lead compounds through structure-activity relationship (SAR) studies to improve potency, selectivity, and pharmacokinetic properties. Finally, the most viable candidates undergo preclinical development to assess safety and efficacy in vitro and in vivo before entering clinical trials.

Various approaches are utilized in drug discovery:

  • Target-based drug discovery: Focuses on designing molecules that interact specifically with a known biological target involved in a disease pathway.
  • Phenotypic drug discovery: Involves screening compounds for their ability to produce a desired physiological effect in a whole cell or organism, without requiring prior knowledge of the molecular target.
  • Structure-based drug design (SBDD): Uses computational tools like X-ray crystallography or NMR to visualize the 3D structure of the target, allowing for the rational design of ligands that fit the binding site.
  • Fragment-based drug discovery (FBDD): Identifies small chemical fragments that bind weakly to the target, which are then linked or grown into larger, more potent molecules.
  • Natural product-based discovery: Derives lead compounds from plants, marine organisms, or microorganisms, which often possess complex chemical scaffolds.

Pharmacological studies are critical in drug discovery for several reasons:

  • Efficacy Determination: These studies confirm that the lead compound produces the intended therapeutic effect in biological models, establishing the proof-of-concept.
  • Dose-Response Relationship: Pharmacological assessment helps establish the minimal effective dose and the therapeutic window, which are essential for clinical trial design.
  • Mechanism of Action (MoA): Understanding how a drug interacts with its target at a molecular level ensures that the drug acts through a predictable and safe pathway.
  • Safety Profiling: Early pharmacological screening identifies potential off-target effects and toxicities, allowing for the early termination of compounds that pose significant safety risks, thereby saving time and resources.

Discuss the role and responsibilities of a sponsor and investigator during a clinical trial.

14 marks

Answer

In a clinical trial, the sponsor and the investigator hold distinct yet complementary roles defined by Good Clinical Practice (GCP) guidelines to ensure the protection of human subjects and the integrity of trial data.

The Sponsor is an individual, company, institution, or organization that takes responsibility for the initiation, management, and financing of a clinical trial. Their key responsibilities include:

  • Protocol Development and Quality Assurance: Designing the study protocol, selecting qualified investigators, and implementing quality assurance and quality control systems.
  • Regulatory Compliance: Ensuring the trial is conducted according to the protocol, GCP, and applicable regulatory requirements, including obtaining necessary approvals from regulatory authorities and Ethics Committees.
  • Supply Chain Management: Providing the investigational product (IP), ensuring it is manufactured according to Good Manufacturing Practice (GMP), and maintaining proper labeling and blinding.
  • Safety Reporting: Monitoring the overall safety of the investigational product and ensuring that serious adverse events are reported to regulatory authorities and all participating investigators.
  • Data Management: Ensuring the accuracy and integrity of data collection, storage, and reporting, and providing adequate monitoring of the trial sites.

The Investigator is the individual responsible for the conduct of the clinical trial at a trial site. If a trial is conducted by a team at a site, the investigator is the responsible leader (Principal Investigator). Their key responsibilities include:

  • Subject Protection: Ensuring that informed consent is obtained from each participant before any trial-related procedures and prioritizing the safety and well-being of subjects throughout the study.
  • Protocol Adherence: Conducting the study strictly according to the approved protocol and ensuring that only authorized staff perform trial-related duties.
  • IP Management: Maintaining accurate records of the receipt, use, and return of the investigational product at the site.
  • Data Integrity: Ensuring that all data recorded in the Case Report Forms (CRFs) is accurate, complete, and verifiable against source documents.
  • Communication and Reporting: Reporting all adverse events to the sponsor as required by the protocol and maintaining regular communication with the Institutional Review Board (IRB) or Ethics Committee regarding the progress and safety of the trial.

Ultimately, the sponsor provides the framework and resources for the research, while the investigator provides the clinical expertise and direct oversight of the trial participants, creating a system of checks and balances necessary for valid clinical evidence.
