Answer any five questions. All questions carry equal marks.

Write a short note on Frerickson’s Classification of Hyperlipidemia and Pharmacotherapy of Hyperlipidemia.
Answer
Fredrickson’s classification categorizes hyperlipoproteinemia into five types based on the elevation of specific lipoproteins: Type I (elevated chylomicrons, high triglycerides), Type IIa (elevated LDL, high cholesterol), Type IIb (elevated LDL and VLDL, high cholesterol and triglycerides), Type III (elevated IDL, high cholesterol and triglycerides), Type IV (elevated VLDL, high triglycerides), and Type V (elevated VLDL and chylomicrons, high triglycerides and cholesterol).
Pharmacotherapy of hyperlipidemia aims to reduce cardiovascular risk by lowering LDL-C and triglycerides while increasing HDL-C. First-line therapy involves HMG-CoA reductase inhibitors (statins), which inhibit cholesterol synthesis. Other agents include fibrates (e.g., fenofibrate) for lowering triglycerides, bile acid sequestrants (e.g., cholestyramine) which prevent bile acid reabsorption, nicotinic acid (niacin) to inhibit VLDL production, and cholesterol absorption inhibitors (e.g., ezetimibe). PCSK9 inhibitors are utilized in refractory cases to enhance LDL receptor recycling.
What is Cardiac Cycle and discuss the phases of Cardiac Cycle.
Answer
The Cardiac Cycle is the sequence of mechanical, electrical, and pressure-related events that occur from the beginning of one heartbeat to the beginning of the next. It consists of two main periods: systole (contraction) and diastole (relaxation).
- Atrial Systole: The atria contract, forcing the remaining blood into the ventricles.
- Isovolumetric Ventricular Contraction: Ventricles begin to contract, closing the AV valves (first heart sound, S1). Pressure rises, but volume remains constant.
- Ventricular Ejection: Ventricular pressure exceeds arterial pressure, opening the semilunar valves and ejecting blood into the aorta and pulmonary artery.
- Isovolumetric Ventricular Relaxation: Ventricles relax, pressure drops, and semilunar valves close (second heart sound, S2). Volume remains constant.
- Ventricular Filling: AV valves open, and blood flows passively from the atria into the ventricles, completing the cycle.
