Clinical study design – Interventional studies

7 marks

Answer

Interventional studies, or clinical trials, are experimental designs where researchers actively manipulate variables to assess the efficacy and safety of a treatment or intervention. These studies are characterized by the prospective assignment of participants to intervention and control groups to minimize bias.

Key types include:

  • Randomized Controlled Trials (RCTs): The gold standard, where participants are randomly assigned to either the experimental group (receiving the new intervention) or the control group (receiving a placebo or standard care). Randomization eliminates selection bias.
  • Non-randomized trials: Studies where participants are assigned to groups without randomization, often used when randomization is ethically or logistically impossible.
  • Blinding: Single-blind (participant unaware), double-blind (participant and researcher unaware), or triple-blind (participant, researcher, and data analyst unaware) designs are used to prevent observer and performance bias.
  • Crossover designs: Participants receive both the intervention and control treatment in sequence, with a washout period in between, allowing each participant to serve as their own control.

These studies are essential for establishing causality between an intervention and a clinical outcome.

b

Determination of sample size.

7 marks

Answer

Sample size determination is the process of calculating the minimum number of participants required to detect a clinically significant effect with a specified degree of confidence and power. An inadequate sample size may lead to type II errors (false negatives), while an excessively large sample wastes resources and exposes unnecessary subjects to risk.

The calculation depends on four primary parameters:

  • Effect Size: The minimum clinically important difference (MCID) that the study aims to detect.
  • Alpha (α): The significance level, typically set at 0.05, representing the probability of a type I error (false positive).
  • Beta (β): The probability of a type II error. Statistical power is defined as 1 - β, usually set at 0.80 or 0.90.
  • Standard Deviation (σ): The expected variability of the outcome measure in the population.

For a continuous variable, the basic formula for sample size (n) per group is:

n = 2 * (Zα/2 + Zβ)² * σ² / Δ²

Where Zα/2 is the critical value for the significance level, Zβ is the critical value for power, and Δ is the effect size. Software tools or power analysis tables are commonly used to solve these equations based on the specific study design, such as superiority, non-inferiority, or equivalence trials.
